Do I register once, or for every property?
Both. You register yourself once, then add each property. You get a number for yourself and one for each property, and the government says the fee is £65 per property per year.
The detail
You register yourself once (a landlord entry) and then each property (a property entry). You can only add a property once your landlord entry is active. You get a unique number for yourself and one for each property, and the fee is paid per property.

The government says the fee is £65 per property per year, paid each time you register or renew a property. During the rollout the fee will be pro-rated, so registering early does not cost you more.
Note: The regulations do not fix the amount; the database operator sets it, so it can change (for example with inflation). The exact pro-rated amounts have not been published.

Adverts for a let property will need to show the landlord and property registration numbers. The government says it will publish guidance before this requirement starts.
