Do I need to register an HMO?
Yes. An HMO let on assured tenancies must be registered like any other rental property. The property entry also asks whether you need an HMO, additional or selective licence, and for the licence numbers.
The detail
HMOs let on assured tenancies must be registered like any other let property. The property entry also asks whether it needs an HMO, additional or selective licence and, if so, the licence numbers, and how many households live there.
Note: The rules do not yet say whether a house let room by room needs one property entry (and one fee) for the whole house or one per room.

The duty to register applies to private landlords of assured tenancies and regulated (Rent Act 1977) tenancies in England. Social housing is not included.

The government says the fee is £65 per property per year, paid each time you register or renew a property. During the rollout the fee will be pro-rated, so registering early does not cost you more.
Note: The regulations do not fix the amount; the database operator sets it, so it can change (for example with inflation). The exact pro-rated amounts have not been published.
